digiChart®.
digiChart ® is an internet based electronic medical record. An internet based electronic medical record allows healthcare providers to access patient information whenever and wherever they have access to the internet. Please see the HIPAA and Patient Confidentiality page to read about the safety of patient information.
Why should I complete my medical history through the digiChart® Patient History portal?
The patient history portal enables patients to communicate their medical history from the comfort of their home or office at their own pace prior to patient's appointment. Completing the medical history online will save time by organizing the patient's history information prior to the appointment and enhance discussion with the healthcare provider about the patient's current health and healthcare needs at the time of the appointment.
When you make an appointment as a new OB , new GYN, referral, or established GYN wellness visit, you will be forwarded a letter with a username and password for access to the patient portal. You will not be able to access the patient portal before you make an appointment or 24 hours after your appointment.
