DigiChart® Electronic Patient Portal Instructions

To access the digiChart® Patient Portal in order to enter your medical history is easy. Just follow the instructions below:

Step 1 If you have access to a computer that has internet service, go to Step 2.

If you do not have internet service, you cannot enter your medical history through the patient portal. Call Central Carolina Women's Center at 336-626-6371.

Step 2 If the computer you are using has the internet browser Microsoft Explorer version 5.0 or higher, go to Step 3.

If you do not Microsoft Explorer version 5.0 or higher, you cannot enter your medical history through the patient portal. Call Central Carolina Women's Center at 336-626-6371.

Step 3 Connect to the internet through Microsoft Explorer and enter the following enter the following in the address bar EXACTLY as shown: https://patients.digichart.com.

(Do not type “www” in front of or after https). Go to Step 4 .

Step 4 At the digiChart® Patient Portal, type the following username and password EXACTLY as shown and click Login:

Username: (refer to your letter from CCWC)

Password: (refer to your letter from CCWC)

Go to step 5.

Step 5 You will be prompted to enter your e-mail address as a new username . This is not an error. It is an added security feature. If you do not have an e-mail address, enter your first name and last name with no spaces @email.com. Ex. janedoe@email.com Go to step 6.

Step 6 You will be prompted to enter a new password . This is not an error. It is an added security feature. Please choose a password that you can remember. The password must be at least 6 characters. The 6 characters can be a combination of letters and numbers. Go to Step 8

Step 7 You will now be prompted to choose a secret question from a list. Please enter the answer as directed. Go to Step 9.

Step 8 You will be prompted to read a privacy statement from digiChart®, our electronic medical records vendor digiChart®. In summary, the statement ensures your medical information is private and only viewable by CCWC physicians and staff. Please accept and go to Step 7.

Step 9 You will be shown the date of your appointment. Please click on the date of your appointment. You are ready to begin the medical history questionnaire.

You can complete the medical history anytime after the date of this letter up to 24 hours after your appointment at which point you will not be able to access the digiChart® Patient Portal. It will take you approximately 30 minutes to complete your medical history. Try to choose a time to complete the history that is uninterrupted. If you are interrupted while entering your information, you can return to it at your convenience and pick up where you left off.
